Comprehensive Medicine Review


A CMR entails:  

1.  reviewing medical history, treatments and lab results 

2.  addressing any questions or concerns

3.  creating a Care Plan.

A Care Plan includes:

1.  a comprehensive list of medicines and supplements

2.
 
a summary of concerns addressed during the visit

3.
 
any recommendations made to you or your healthcare provider to improve or monitor the efficacy and safety of a treatment

4.  educational resources to assist in managing an ongoing health condition or treatment, or for prevention of an undesirable condition, complication or side-effect.



Any letters of recommendation written to your healthcare provider may be brought to your provider’s office for discussion during your next office appointment.  They may also be sent to your provider via regular mail or faxed before the appointment.

he success of a recommendation may depend on your prescriber –recommendations that require a change to a prescription or a new prescription must come from your doctor or prescriber.  Medicine Reviews PLLC works to ensure that all of the evidence favoring an initiation of a new medicine, a change to an existing medicine, or additional monitoring services, such as ordering a lab assessment, is presented within the recommendation.  In some instances, a copy of a recommendation can be provided to your provider, which he or she may then use to initiate a change, without requiring an office visit. 

While s
ome physicians gratefully welcome recommendations, some do not.  There is no guarantee that a recommendation will be accepted or reviewed.  In cases where important recommendations are not reviewed or accepted, a recommendation may be made to seek a new provider to help ensure optimum healthcare.    

Once a scheduled
visit is complete, you have the option to receive a follow-up visit within two to four months, depending on the complexity of conditions, treatments, recommendations previously made, the number of concerns addressed during the initial visit and those remaining.
